Overview of pharmaceutical excipients used in tablets and capsules




The pharmaceutical industry is ever thirsty to satisfy patient’s therapeutical needs and apart from active ingredients, inactive excipients play a major role in formulation development. Pharmaceutical excipients are substances other than the pharmacologically active drug or prodrug which are included in the manufacturing process or are contained in a finished pharmaceutical product dosage form.

In addition to transporting the active drug to the site in the body where the drug is intended to exert its action, excipients play an important part in the manufacturing process. They may also be important for keeping the drug from being released too early in the assimilation process in places where it could damage tender tissue and create gastric irritation or stomach upset.

Others help the drug to disintegrate into particles small enough to reach the blood stream more quickly and still others protect the product's stability so it will be at maximum effectiveness at time of use. In addition, some excipients are used to aid the identification of a drug product.

Last, but not least, some excipients are used simply to make the product taste and look better. This improves patient compliance, especially in children. Although technically "inactive" from a therapeutic sense, pharmaceutical excipients are critical and essential components of a modern drug product. In many products, excipients make up the bulk of the total dosage form (1). Apart from the drug’s active ingredient, other essential components include diluents or fillers, binders, disintegrants, lubricants, coloring agents and preservatives (3). Diluents or fillers are inert ingredients that can significantly affect the chemical and physical properties of the final tablet thus affecting the biopharmaceutical profile.

One classic example of this are calcium salts, which can be utilized as fillers, which interfere with the absorption of tetracycline from the gastrointestinal tract. (4) This example emphasizes that excipients may not always be inert, as they may be perceived.

Usually tablets are designed so that the smallest tablet size which can be conveniently compressed is formed. Thus, if the dose is small more diluents are required and if the dose is high less diluents are required as not to increase the tablet size, which might make it difficult to swallow.

Diluents selection should be made carefully as physical-chemical changes might render the product unstable and might cause problems in manufacturing. Binders are added to tablet formulations to add cohesiveness to powders thereby providing the necessary bonding to form granules which under compaction form a compact mass as tablet. In other words, binders are essential to achieve the “hardness” of the tablet.

Binders are usually selected on basis of previous experience, particular product needs, literature or vendor data or the preference of individual scientists or manufacturing unit. The primary criterion when choosing a binder is its compatibility with other tablet components.

It must add sufficient cohesion to the powders to allow for normal processing yet allow the tablet to disintegrate and the drug to dissolve upon ingestion, releasing the active ingredients for absorption. Disintegrants facilitate the breakup of a tablet after oral administration.

They can be added prior to granulation or during the lubrication step prior to compression or at both processing steps. The effectiveness of many disintegrants is affected by their position within the tablet. Since disintegration is the opposite operation to granulation (agglomeration) and the subsequent formation of strong compacts, one must carefully weigh these two phenomena when designing a tablet. Lubricants prevent sticking of the tablets to the tablet punches during the compression phase of the tablet manufacturing process.

When lubricants are added to a powder mass, they form a coat around individual particles which remains more or less intact during compression. Lubricants are mostly hydrophobic. The presence of lubricant coating may cause an increase in the disintegration time and a decrease in drug dissolution rate. The choice of a lubricant may depend upon the type of tablet being manufactured, dissolution, flow characteristics and requirements of the formulation in terms of hardness, friability and compatibility. Glidants are the materials that have good flow properties and poor lubrication properties.

Glidants improve the flow of powder into the tableting machines for compaction. They act to minimize the tendency of a granulation to separate or segregate due to excessive vibration. High speed tablet machine require smooth even flow of material to die cavities (tablet mold). The uniformity of tablet weights directly depends on how uniformly the die cavity is filled. In general many materials commonly referred to as lubricants possess only a minimal lubricating activity and are better glidants or anti-adherents.

Thus a blend of two or more materials may be necessary to obtain these properties. Pharmacists should be familiar with the components of pharmaceuticals products, beyond their active ingredients. In order to educate pharmacists on excipients that are routinely used in the pharmaceutical industry, we decided to examine the top 200 prescription tablets and capsules products of 2003 (2) and find out how many or which excipients are used in each product.

The selection will cover both brand and generic drugs. Out of the 200 prescription drugs, the total numbers of inactive excipients used except for coating and coloring agents were ONLY 94! Although the list is composed on the top 200 drugs of 2003, very few blockbusters has been launched since then and still the excipients in all remains the same.

From the table above we see that most of the commonly used ingredients have not been changed in making a tablet from generations and might follow the same trends for many generations to come.

CONCLUSION: Lot of time and effort are still needed in field of excipients. However till then a formulation scientist is entrusted with the limited amount of excipients and will have to deal with 94 excipients or now maybe 104.

Abiotic Degradation
Breakdown of a chemical by processes other than living organisms, such as photodegradation and chemical reactions (e.g. hydrolysis).

Absorbed Dose

Adsorption
The process of attracting and holding a substance to a surface. For example, a substance may adsorb onto a soil particle.

Adverse Reactions to Drug Report
A report which is voluntarily submitted by physicians to the FDA after a drug has been approved and in use.

Aerosols
Aerosols are airborne particulates. They may be solids or liquid droplets.

Allergy
An immune hypersensitivity reaction of body tissues to allergens that can affect the skin (urticaria), respiratory tract (asthma), gastrointestinal tract (vomiting and nausea) or produce a systemic circulatory response (anaphylactic response).

Alveoli
The air sacs at the ends of the tracheo-bronchial tree in which gases are exchanged between inhaled air and the pulmonary capillary blood.

Autoimmunity
An immune response that recognizes the constituents of the body's own cells as foreign and thus induces hypersensitivity to its own tissues.

Average Daily Intake
The amount of a chemical to which a person consumes over a period of a day. It is determined by multiplying typical concentration of the chemical in drinking water, air, and food by
an average daily intake factor such as 2 liters of water per day.

A tumor that grows only at the site of origin; and does not invade adjacent tissues or metastasize. It is generally treatable.

Bias
Systematic error that may be introduced in sampling by selecting or encouraging one outcome over another.

Bioaccumulation
The build-up of a substance in a biological organism such that the level in the organism is greater than in environmental source of the substance. same as Bioconcentration.

Bioactivation
The metabolic process whereby a parent substance is chemically changed to a daughter substance with enhanced biological activity.

Bioassay

Biomarker
Indicators of events occurring in biologic systems due to a xenobiotic. The types of indicators are exposure, effects, or susceptibility.

Biotransformation
Conversion of a chemical from one form to another by a biological organism.

Body Burden
The concentration of a substance which has accumulated in the body.

Bone Marrow
The tissue within the internal open space of bones (e.g., shaft of long bones) in which the blood-forming elements exist.

Bronchioles
The very small branches of the tracheo-bronchial tree of the respiratory tract which terminate in the alveoli.

Cancer
An uncontrolled growth of abnormal cells, creating a tumor that can invade surrounding tissues and may spread (metastasis) to distant organs.

Cancer Slope Factor
A key risk assessment parameter derived by the EPA. It is an estimate of the probability that an individual will develop cancer if exposed to a specified amount of chemical (mg/kg) every day for a lifetime.

Carcinogen
A compound which is capable of causing cancer.

Carcinogenic
The ability of a substance to cause cancer.

Carcinogenicity
The complex process whereby normal body cells are transformed to cancer cells.

Case-Control Study
A type of epidemiology study to investigate the cause of the toxicity. It compares the exposure histories of humans who have a particular toxic effect with that of normal individuals.

Ceiling Level
The maximum allowable concentration of a chemical in the workplace for a specific period of time (usually 15 minutes).

Cell Transformation
The change of a cell from one form to another. The term is generally used to denote the change from normal to malignant.

Cholinergic Effect
Neurological effects resulting from the transmission by acetylcholine of impulses across synaptic junctions between nerves.

Cholestasis
A liver condition in which excretion of bile salts via the bile duct is inhibited resulting in bile salts backing up into liver cells.

Chromatography
An analytic method to separate and identify the components of a complex mixture by the differential movement through a two-phase system. The movement is effected by a flow of a liquid or a gas (mobile) phase, based on the physicochemical principles of adsorption, partition, ion exchange, exclusion, or a combination of these principles.

Chromosome
One of a group of structures that form in the nucleus of a cell during cell division. Chromosomes, bearing the DNA, carry the genetic code for the organism.

Chromosome Aberration
Changes in chromosome structure.

Chronic Dose
Substance administered or received gradually over a long period of time (months to years).

Chronic Effect
An effect that either shows up a long time after an exposure (the latency period) or an effect that results from a long term (chronic) exposure.

Cirrhosis
A chronic condition of the liver in which liver cells are replaced by fibrous cells.

CNS
The central nervous system consisting of the brain and spinal cord.

Cohort Study
An epidemiology study in which a cohort (group) of individuals with exposure to a chemical and a cohort without exposure are followed over time to compare disease occurrence.

Conjugation
A metabolic process in which chemical groups are attached to foreign substances in the body, usually making the conjugated chemical more water soluble and easier to eliminate from the body.

Control Group
A group of animals or humans in a study that are treated the same as the exposed groups but without receiving the specific exposure.

Cornea
The transparent front surface of the eye.

Corrosion
Direct chemical action that results in irreversible damage at the site of contact. It is manifested by ulceration, necrosis, and scar formation.

Covalent Bond
The joining together of atoms that results from sharing electrons.

CPSC
Consumer Product Safety Commission. It is a federal agency responsible for protecting the public from toxins and other hazards present in consumer products.

Cross-Sectional Study
A type of epidemiology study that tests for the prevalence of a disease or clinical parameter among one or more exposed groups (e.g., the prevalence of respiratory conditions among furniture makers).

Dermal Sensitization Test
An assay for immune hypersensitivity of the skin.

Dermal Toxicity
Toxicity of the skin which can range from mild irritation to corrosivity, hypersensitivity, and skin cancer. It can result from direct contact or internal distribution of the xenobiotic to the skin.

Detoxification
A metabolic process whereby a parent substance is changed to a daughter product (metabolite) that has lessened toxicity.

Disposition
The term used to describe the kinetics of a substance in the body. It encompasses absorption, distribution, metabolism, and elimination of a chemical.

Distribution
Movement of a substance from the site of entry to other parts of the body.

DNA
Deoxyribonucleic acid. The very large molecules in the nucleus of the cell, that carry the genetic information.

Dominant Lethal Assay Test
A mutagenicity test that can detect heritable dominant lethal mutations present in the sperm as the result of exposure to a substance.

Dosage
The determination of quantity of a substance received that incorporates the size, frequency, and duration of doses (e.g., 10 mg every 8 hours for 5 days).

Dose
The amount of a substance received at one time. Dose is usually expressed as administered or absorbed dose (e.g., milligrams material/kilogram of body weight).

Dose-Response Assessment
The relation between dose levels and associated effects.

Dose-Response Curve
A graphical representation of the quantitative relationship between doses of a substance and specific biological effects.

DOT
Department of Transportation. A federal agency responsible for the safe shipment of toxic chemicals and other hazardous materials.

Draize Test
The test for eye irritation in which the test substance is placed on the eyes of white rabbits and observed for 72 hours.

Drug Idiosyncrasy
An abnormal susceptibility or sensitivity to a drug.

Drug Tolerance
The unusual ability to endure, without effect, a quantity of a drug that would normally produce biological activity.

Ecologic
A term referring to the general environment.

Ecotoxicity
The toxic effects on environmental organisms other than humans.

ED50
Effective dose 50%. The estimated dose that causes some specific effect (usually desirable) for 50% of the population.

ED99
Effective dose 99%. The estimated dose that causes some specific effect (usually desirable) for 99% of the population.

Edema
The retention of fluid in an organ or in the body.

Effluent
The discharge of waste from a plant or other source into the environment.

Embryo
An early stage of the development of the unborn offspring in which cell differentiation proceeds rapidly along with the formation of the major organs. In humans this stage occurs from about 3 weeks until 8-9 weeks after conception.

Embryotoxic
The harmful effects of a substance on the developing embryo.

Environmental Fate
The fate of a substance following its release into the environment. It includes the movement and persistence of the substance.

Enzyme
A protein which serves as a catalyst for chemical reactions in cells.

Enzyme Activation
The increase in levels of an enzyme as the result of stimulation by another chemical substance.

Enzyme Inhibitor
A substance which causes a decrease in levels of an enzyme.

EPA
Environmental Protection Agency. A federal agency responsible for regulation of most chemicals that can enter the environment. The EPA administers the following acts: Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA), the Toxic Substances Control Act (TSCA), the Resource Conservation and Recovery Act (RCRA), the Safe Drinking Water Act (SDWA), Clean Air Act (CAA), and the Comprehensive Environmental Response, Compensation and Liabilities Act (CERCLA) ( Superfund Act).

Epidemiology
The study of the relative characteristics of exposed and nonexposed human populations for the purpose of detecting harmful effects.

Epidermis
The outer layer of the skin.

Excretion
A process whereby substances (or metabolites) are eliminated from the body.

Exposure

يجب أن يكون لدى المؤسسات الصحية تعريفاً يُعين التأثير العكسي للدواء، وبذلك تتمكن من كشفه، وتقييمه وكتابة التقرير فيه بشكل واضح.

أمثلة على التعاريف الأكثر شيوعاً:



تعريف الجمعية الأمريكية لصيادلة المنظومة الصحية (ASHP) : التأثير العكسي الهام للدواء هو: " أية استجابة غير مقبولة، أو غير مطلوبة ، أو غير مرجوّة، أو مُفرطة للدواء ويؤدي إلى أحد الاحتمالات التالية:

1. توقيف الدواء ( العلاجي أو التشخيصي).

2. تغير العلاج الدوائي.

3. تعديل الجرعة (تُستثنى التعديلات البسيطة).

4. الاضطرار للدخول إلى المستشفى.

5. إطالة زمن المكوث في المستشفى.

6. الاضطرار إلى العلاجات الداعمة.

7. تعقيد واضح للتشخيص.

8. تأثير سلبي على تحسن حالة المريض.

9. ينتج عنه ضرر دائم أو عجز أو وفاة."



تعريف منظمة الصحة العالمية WHO :التأثير العكسي للدواء هو :" أية استجابة للدواء ضارة أو غير مطلوبة والتي تحدث عند تناول الإنسان للجرعات الدوائية التي توصف عادة لعلاج أو وقاية أو تشخيص المرض ، أو لتعديل الوظيفة الفيزيولوجية".



تعريف إدارة الغذاء والدواء الأمريكية FDA : عرفت التأثيرات العكسية الخطيرة للأدوية أو الأجهزة على أنها كل ما ينتج عنه موت المريض، أو تعريض حياته للخطر، أو إدخاله إلى المستشفى أو إطالة إقامته فيها، أو إصابته بالعجز الشديد أو المقاوم أو الدائم، أو يؤدي إلى تشوه خَلقي، أو يتطلب التدخل للوقاية من حدوث تلف أو ضرر دائمين".



التأثيرات العكسية الممكن تفاديها :" هو تفاعل مؤذي أو غير مستساغ يمكن تقديره، ناتج عن طارئ له علاقة بتناول مُنتج دوائي، مما يتطلب توقع الخطر من تناوله مستقبلاً، أو الحذر منه، أو الاضطرار لتناول علاجاً مضاداً، أو تعديلاً للجرعة أو سحب المنتج من السوق".



تعريف كارش و لازانيا Karch and Lasagna: " أية استجابة ضارّة للدواء أو غير مطلوبة، والتي تحدث للشخص عند تناوله الجرعات الدوائية الموصوفة للوقاية أو التشخيص أو العلاج، يُستثنى من هذه التأثيرات الفشل في بلوغ الهدف من تناول الدواء".



برنامج التأثيرات العكسية للدواء:



يجب على المؤسسات الصحية تأسيس برنامج للتأثيرات العكسية مما يُسهل مراقبتها وتقييمها وتدوينها وكتابة التقارير عنها، كما تتيح هذه البرامج التعليم للأطباء وموظفي الرعاية الصحية والمرضى. بالإضافة إلى ذلك يجب على هذه البرامج أن تُركز على تعريف التأثيرات العكسية الممكن تفاديها، وتحديد العوامل المرتبطة بحدوثها، وتطوير الاستراتيجيات لتلافي وقوعها في المستقبل.



يجب أن تُضم النتائج والمعلومات المتعلقة ببرنامج التأثيرات العكسية للدواء إلى نشاطات تحسين الجودة وإدارة المخاطر في المؤسسات الصحية.



يُدار برنامج التأثيرات العكسية نموذجياً ويوجه بعناية من قبل لجنة الصيدلة والعلاج، واللجنة التنفيذية للهيئة الطبية، وإدارة المؤسسة الصحية، وعادة ما يقوم قسم الصيدلية بمراقبة و صيانة البرنامج من حيث جمع البيانات، وتحليلها ورفع التقارير عنها.





كشف ورصد التأثيرات العكسية للأدوية:
يمكن للمراجعة المنتظمة والمستمرة للعلامات والأعراض الخاصة بالمريض، أن تُساعد كثيراً على تحديد التأثيرات العكسية للأدوية، إلاّ أنه يجب على العاملين في الرعاية الصحية أن يكونوا على علم كاف يُمكنهم من التفريق بين التأثيرات العكسية للأدوية عند حدوثها وبين تطور الحالة المرضية.

يمكن رصد التأثيرات العكسية للأدوية بثلاثة طرق إما برصدها بأثر رجعي retrospective أو الرصد المتزامن concurrent أو بالرصد الاستباقي لحدوثها prospective.



الرصد بأثر رجعي: تكون بعد حدوث التأثيرات العكسية ، ويتم ذلك بمراجعة السجلات الطبية للبحث عن التأثيرات العكسية والتشاخيص الطبية المدونة. وتعتبر هذه الطريقة غير كافية لأن تسجيل التأثيرات العكسية للأدوية في السجلات الطبية قد لا يكون كافياً أو كاملاً في أغلب الأحيان.



الرصد المتزامن: يتم خلال فترة المعالجة على أمل تقليل التأثيرات الغير مرغوب بها التي قد يسببها العلاج، ومراقة التكلفة التي تصاحبها. وتشتمل هذه الطريقة على متابعة إشارات محددة كالتالى:

o نتائج التحاليل المخبرية (مثل ارتفاع مستوى بعض الأدوية في الدم مثل الأماينوغلايكوزايد aminoglycosides ، وارتفاع في نتائج التحاليل المتعلقة بالتخثر).

o توقيف الدواء المفاجئ.

o الأمر الطبي السريع بإعطاء ترياق مضاد، مثل فيتامين ك، أو مضادات الهيستامين، أو الستيروئيدات، أو الدكستروز.

o طلب تحاليل مخبرية غير روتينية مثل مستوى البولينا في الدم (BUN)، مستوى الدواء في الدم، مستوى الكرياتينين، زمن التخثر وغيرها.



الرصد المسبق: وهو المفضل حيث يمكن تفادي وقوع هذه التأثيرات أو تقليلها. وعادة ما يشتمل على رصد الأدوية التي ينطوي استعمالها على خطر عالٍ لحدوث تفاعل عكسي مثل الأماينوغلايكوزايد aminoglycosides، والعلاجات الكيماوية (مضادات السرطان) chemotherapeutics، والمخدرات narcotics ، ومُضادات التخثر thrombolytics ، أو مراقبة ورصد المرضى الذين لديهم الخطر عالياً لحدوث التأثيرات العكسية كالأطفال وكبار السن ومرضى الفشل الكلوي والكبدي والذين يتناولون أدوية متعددة.



تدوين التأثيرات العكسية للأدوية:

يجب تدوين كل تفاعل عكسي متوقع والنتائج الحاصلة في السجل الطبي للمريض، وأن تُبلغ إلى كل المعنيين برعاية المريض بما في ذلك المريض نفسه.



تقييم التأثيرات العكسية للأدوية، وتعيين الاحتمالات:

يجب تقييم الأسباب المتعلقة بكل تفاعل عكسي مُحتمل الحدوث بالاعتماد على تاريخ المريض الطبي والعلاجي، والظروف التي أدت إلى حدوث هذا التفاعل ، ونظريات علم أسباب الأمراض البديلة، ومراجعة المقالات الطبية.



بما أن تقارير التأثيرات العكسية للأدوية في غالبيتها تقوم أساساً على أشياء مترابطة مؤقتة أو محتملة ، لذا تستعمل أكثر المؤسسات الصحية أدوات خاصة لتقييم الاحتمال الأكثر رجوحاً لحدوث هذه التأثيرات الواردة في التقارير أو ما يسمى ، إحتمالية أن يحدث الدواء التأثير المعاكس .



تتصف أداة التقييم المثالية بكونها موثوقة وسهلة وسريعة الاستعمال. ومن بين الأدوات المستعملة كرامر ألغوريزم Kramer Algorithm ، جونز ألغوريزم Jones Algorithm ، نارنجو ألغوريزم Naranjo Algorithm وتعتبر الأخيرة هي الأكثر استعمالاً، فهي تحتوي على عشرة بنود استبيانية تستعمل قيم رقمية معينة بشكل خاص لتصل بمجموع نقاطها إلى تعيين الاحتمال الأرجح.

يُعبر عن الاحتمال الأرجح بدرجة معينة بمصطلح كالتالي: مؤكد، مرجح، محتمل، مريب. تتميز أداة نارنجو ألغوريزم بسهولة وانتشار استعمالها ، وعلى كلٍ فإن التجارب على فاعليتها كأداة لتقييم الحالات الخطيرة من التأثيرات العكسية للدواء لم تُجرَ بعد.



كتابة التقارير عن التأثيرات العكسية للأدوية
يجب رفع التقارير عن التأثيرات العكسية للأدوية وخصوصاً إذا كانت غير متوقعة أو خطيرة أو حدثت بعد تناول دواء جديد التسويق أو تمت المصادقة عليه مؤخراً. يُمكن للتقارير أن تشتمل على الأدوية الموصوفة والغير الموصوفة، والأدوية البيولوجية بما فيها منتجات الدم، والأدوية البديلة بما فيها الأعشاب، والمنتجات الصيدلانية التي تستعمل في الطب الشعاعي.



يمكن للمؤسسات الصحية أن تطور استمارات خاصة لكتابة التقارير يُعتقد بأنها قابلة للتطبيق فيها بحسب خلفية أنظمتها، أو يمكنها أن تستعمل نماذج التقارير الخاصة بالمؤسسات الصحية الدولية الأخرى.

بشكل عام يجب أن تكون الاستمارة بسيطة ، ومفهومة وتحتوي على العناصر التالية:

أُُسس الكثير من البرامج الدولية لكتابة التقارير لفتح المجال أمام العاملين في الرعاية الصحية وغيرهم لرفع التقارير عن التأثيرات العكسية الخطيرة للأدوية.

إنّ رفع التقارير عن هذه المشاكل المتعلقة بالدواء يُقدم خدمة جليلة للصحة العامة حيث أنها تقوم بتنبيه العاملين في الرعاية الصحية عن المشاكل والحوادث التي يمكن تلافي حدوثها للمرضى الآخرين في المستقبل.

تعاريف :



الخطأ الدوائي بحسب تعريف المجلس الوطني الأمريكي لتنسيق تسجيل الأخطاء الدوائية وتجنب حدوثها The National Coordinating Council for Medication Error Reporting and Prevention NCCMERP:

" هو أي حادث يمكن تجنبه، والذي قد يسبب أو يؤدي إلى استعمال غير ملائم للدواء أو إلى وقوع ضرر ما على المريض، في الوقت الذي يكون فيه الدواء تحت رقابة المسؤولين في الرعاية الصحية أو المريض أو المستهلك. ويتعلق هذا النوع من الحوادث بكل ما يخص الدواء من عمليات تداول فقد يكون متعلقاً بالمزاولة المهنية، أو بما تقدمه الرعاية الصحية من خدمات ومنتجات ، أو بالإجراءات المُتبعة، أو بالأنظمة. ويشمل ذلك وصف الدواء، وطريقة تداول الوصفات الطبية (الأوامر الطبية للمرضى المنومين) بين الأشخاص المعنيين ( الممرض والصيدلي والفني وغيرهم) ، واللصاقات الموجودة على عبوات الأدوية ، وتعبئتها، وأسماءها ، كما يشتمل على طريقة تحضير الأدوية ، وصرفها، وتوزيعها، وإعطاءها للمريض، وتعليم المريض ،و الرقابة، والاستعمال".



الأخطاء الدوائية : هي أخطاء تقع في أي مرحلة من مراحل عملية تداول الدواء ( وصفه، نسخه، توزيعه، إعطاؤه، مراقبته) والذي قد يؤدي أو لا يؤدي إلى نتائج عكسية.



أنواع الأخطاء الدوائية:



الأخطاء في الوصفة:



ينشأ هذا الخطأ عن الكتابة الخاطئة للأمر الدوائي ( الوصفة )، فقد تكون الوصفة غير مقروءة أو غير واضحة أو يتعلق الخطأ باسم المريض ، أو الدواء أو الجرعة أو طريق الاستعمال أو جدول تكرار الجرعات.



الأخطاء في نسخ الوصفة :



ينشأ هذا الخطأ خلال نسخ الأمر الدوائي في سجل إعطاء الدواء Medication Administration Record MAR. وقد يقع الخطأ في النسخ على اسم المريض أو الدواء او أو طريق الاستعمال أو جدول تكرار الجرعات.



الأخطاء في توزيع وصرف الدواء:



ينشأ هذا الخطأ بداية من الصيدلية حيث يتم تحضير الدواء ، فقد يحدث الخطأ في إرسال الدواء إلى المريض الخطأ، أو يرسل الدواء الخطأ، أو يحدث الخطأ في الجرعة المرسلة أو طريق الاستعمال ، أو في جدول تكرار الجرعات .



الأخطاء في إعطاء الدواء:



ينشأ هذا الخطأ أثناء عملية إعطاء الدواء للمريض ، فقد يقع الخطأ عند إعطاء الدواء لمريض آخر،أو يُعطى الدواء الخطأ، أو يحدث الخطأ في الجرعة أو طريق الاستعمال ، أو جدول تكرار الجرعات، أو قد يُغفل عن إعطاء الدواء أصلاً.



الأخطاء في الرقابة :



ينشأ هذا الخطأ عن نقص في الرقابة الضرورية. ويشمل هذا النوع من الأخطاء الرقابة الضرورية التي لم يؤمر بها، أو لم تنجز ،والتي لم يُعمل بها.




خطورة الأخطاء الدوائية:



استُعملت أنظمة قياس مختلفة لتصنيف أو تبويب درجات خطورة الأخطاء الدوائية، حيث استعمل البعض المقياس الرقمي مثل ( مستوى صفر إلى مستوى 6) أو المقياس الأبجدي مثل ( مستوى أ إلى مستوى ياء) أو المقياس الكلمي مثل (قريب من الخطأ، ضئيل إلى الموت). وتتراوح كل المقاييس السابقة بين عدم وجود خطأ أو احتمال وجود خطأ إلى الموت.

وتساعد المقاييس في تتبع وتحليل الأخطاء الدوائية، وتأثيرها على المرضى.

يمكن أن يكون رفع التقارير عن الأخطاء الدوائية إجبارياً في حال حدوث ضرر شديد أو موت، أو اختيارياً كما هو مطبق في الكثير من الهيئات الصحية في العالم.

تحولت الكثير من المؤسسات من استعمال أسلوب أنظمة التقارير المكتوبة على الورق التي عادة ما توحي بلهجة عقابية موجهة إلى أفراد معينين بدلاً من التوجه إلى مجهول، كما أنها تهدر الكثير من الوقت ، إلى أسلوب التقارير الإلكترونية المبنية على استعمال شبكة الانترنيت والتي توفر إخطاراً الكترونياً لأهم مُتعلقات الأحداث يوجه إلى أصحاب القرار، ووصولاً سريعاً إلى المعلومات مع توفير التحليلات الإحصائية.



ولقد أظهرت المقالات المنشورة بأن التقارير الالكترونية قد حسنت من طريقة رفع التقارير عن الأخطاء إلى أبعد الحدود.



وبشكل عام فإن التقارير سواءً كانت مكتوبة أو الكترونية فإنها يجب أن تكون بسيطة وسهلة الاستعمال وتحتوي العناصر التالية:



المعلومات الخاصة بالمريض: وذلك يشمل على اسمه، رقم سجله الطبي، عمره، موقعه، وتاريخ الحادث.
المعلومات الخاصة بالدواء : تصنيفه ، اسمه العلمي والتجاري، الجرعة، التركيز، القوة، طريق الاستعمال، وقت الاستعمال.
نوع الأخطاء الدوائية : عادة ما توضح هذه الفقرة كل أنواع الأخطاء الدوائية ( وصف الدواء، نسخ الوصفة، صرفه، إعطاؤه، المتابعة)، مع وصف تفصيلي وأمثلة عن كل نوع، مما يُمكن القائم على التقرير من اختيار ما يُناسب الحالة..
مستوى خطورة الخطأ الدوائي أو نتائجه على المريض: يجب أن تكون درجات قياس الخطأ محددة بوضوح.


وصف الحادث: يجب أن يتضمن الرد على كل الأسئلة ( أين، ماذا، وكيف وقع؟)، كما يجب أن يكون الوصف موضوعياً، ومختصراً، ولا يحدد أية مسؤولية أو اتهام.
الإجراءات التي أتخزت بعد الحادث مثل إبلاغ الطبيب المسؤول أو المُشرف، ووصف لأي علاج تم إعطاؤه.

العوامل ذات الصلة والأسباب المحتملة: تحتوي أكثر الاستمارات على لائحة مُحددة مسبقاً مبنية على أساس الأخطاء المتعلقة بالنظام والتي يُمكن للتقرير أن يحددها كالعجز في المعرفة الدوائية، وعدم الالتزام بسياسات وعمليات المؤسسة، والعجز في الأدوات، والارتباك، وضغط العمل، واستعمال الاختصارات، العجز في عدد الموظفين، والتشابه في الأسماء أو العبوات، وعدم كفاية التدريب.

التوصيات لتلافي وقوع هذا الخطأ مستقبلاً.

الابتعاد عن الوصفات الغير مكتوبة إلا في الحالات الإسعافية حيث تتعذر كتابة الأوامر الطبية والوصفات.

كتابة الوصفات بخط مقروء وبلغة سهلة. يجب على الأطباء اللجوء إلى نظام كتابة الوصفات الألكتروني المباشر إذا أمكن ذلك.

الابتعاد عن الاختصارات.أصدر معهد ممارسة السلامة الدوائية ISMP لائحة "الاختصارات التي تؤدي إلى أخطاء" ، كما أصدرت اللجنة المتحدة لإجازة هيئات الرعاية الصحية في الولايات المتحدة الأمريكية (JCAHO) جدولا سمته" لا تستخدم الاختصارات".

وجوب احتواء الوصفات على توجيهات الاستعمال والابتعاد عن تعبير " استعمل حسب الارشادات".

وجوب كتابة داعي استعمال الدواء مثال ( للسعال).
كتابة اسم المريض وسنه ووزنه.
كتابة اسم الدواء،و قوته أو تركيزه، وشكله الصيدلاني، والجرعة، وطريقة الاستعمال، والتكرار، ومدة العلاج.
كتابة الجرعة بنظام القياس المتري مثال ( مايكروغرام ، ميلليغرام، غرام..) باستثناء الأدوية التي تستعمل وحدات قياسية كالإنسولين. ويجب أن تكتب كلمة "وحدة " كاملة لا أن تُكتب مختصرة مثل "و".
استخدام الصفر قبل الفاصلة في الجرعات التي تكون أصغر من واحد مثال ( يكتب 0.5 ولا يُكتب 0.5 ).
ترك فراغ بين اسم الدواء والجرعة ووحدة القياس مثال:
(E.g. Inderal 40 mg, NOT Inderal40mg).

استعمال فواصل بين الأصفار للجرعات التي تعتمد على قياس الوحدات والتي تكون أكبر من 1000 وحدة مثال:(100,000 unit, NOT 100000unit).
لا توضع نقطة بعد كتابة وحدة القياس مثال (e.g. mg, NOT mg.).


التوصيات الخاصة بتلافي الخطأ المتعلق بصرف الدواء:



مراجعة الطبيب عند كون الأمر الطبي أو الوصفة مشوش أو غير مقروء.
التحقق من المعلومات المتعلقة بالدواء والواردة في الأمر الطبي أو الوصفة وذلك للتأكد من صحتها وعدم احتوائها على تداخلات دوائية أو تكرار لأي علاج.

التحقق من الوصفة مرة أخرى من قبل صيدلي آخر أو أن يكرر الصيدلي نفسه التحقق من الوصفة مرة ثانية قبل صرفها للمريض وخصوصاً عند احتوائها على أدوية عالية الخطورة مثل الهيبارين Heparin ، الإنسولين Insulin والعلاج الكيماوي Chemotherapeutics. وهناك طرق أخرى للتحقق تشتمل عل استخدام النظام الآلي ( مثال أنظمة شريحة التشفير Bar Coding ).

تُقرأ اللصاقة المكتوبة على عبوة الدواء عند أخذه من مكانه ، وعند إعداد الوصفة وعند إعادته إلى مكانه أو رمي العبوة الفارغة.

التحقق المضاعف لأي عملية حسابية متعلقة بالجرعة.

صرف الأدوية بقدر المستطاع بشكل جرعات جاهزة للاستعمال .

استعمال لصاقات التعليمات الإضافية مثل " للاستعمال الخارجي فقط" و" يُحفظ في البراد" و " رج الزجاجة قبل الاستعمال".

توصيل الأدوية إلى مناطق رعاية المريض في الوقت المحدد المصادق عليه من قبل سياسات المستشفى، والاتصال بالممرض المسؤول في حال تأخير الجرعة.

مراجعة الأدوية المرتجعة لاكتشاف الأدوية التي لم تُعطى أو الغير مُصرح بها

تعليم المرضى الخارجيين بخصوص أدويتهم وذلك يتضمن دواعي وطريقة الاستعمال، والآثار الجانبية، والتداخلات بين الدواء والطعام، وطريقة حفظ الدواء، والنتائج المرجوة.

لزوم مشاركة الصيدلي في إعداد الوصفات والأوامر الطبية والاستمارات المسبقة الطبع التي ستستعمل في المستشفى، والموافقة عليها.
أخذ تشابه شكل الأدوية بعين الاعتبار قبل ترتيبها في أمكنتها ، وليس فقط حسب الترتيب الهجائي.

منطقة إعداد الدواء يجب أن تكون مُصممة بشكل يريح العاملين.

تحسين نوعية وعدد العاملين للوصول إلى ضغط عمل معقول.

تدريب وتوجيه العاملين في الصيدلية على مواصفات الممارسة المقبولة المتعلقة بعمليات صرف الدواء مع خفض الخطأ إلى أقصى درجة ممكنة.

يجب جمع البيانات المتعلقة بالأوامر الطبية المستوضحة وتحليلها ورفع التقارير عنها وذلك لتعزيز الطريقة الصحيحة في كتابة الوصفات.


التوصيات الخاصة بتلافي الخطأ المتعلق بإعطاء الدواء:



قبل إعطاء الدواء، يجب أن يكون الممرض على علم بدواعي استعمال الدواء، والنتائج المتوقعة، وآثاره الجانبية، وتداخلاته الدوائية، وشروط حفظه، وتقنية إعطاءه.

يجب على الممرض أن يستوضح أي أمر طبي غير كامل، أو غير مقروء قبل إعطاء الدواء وذلك بحسب السياسة المتبعة في المستشفى.

إذا كانت طريقة نسخ الأوامر الطبية مُطبقة عندها يجب التحقق المُضاعف من العناصر التالية: تاريخ الأمر الطبي، و الاسم الكامل للدواء وشكله الصيدلاني والجرعة، طريق الاستعمال، الجدول الزمني للجرعات، تاريخ بداية ونهاية إعطاء الدواء.

قبل إعطاء الدواء يجب القيام بالتحقق من صحة الأمور الستة التالية: الدواء ، الجرعة ، المريض ، طريق الإعطاء ، الشكل الصيدلاني ، التوقيت، التدوين.

لا يُعطي الممرض إلاّ الأدوية ذات اللصاقات الصحيحة وعليه أن يقرأ اللصاقة أثناء القيام بعملية إعطاء الدواء ثلاث مرات، الأولى عند وصوله أو تحضيره، والثانية قبل إعطاءه مباشرة، والثالثة، عند رمي العبوة أو إعادته إلى مكان الحفظ.

يجب أن لا يتم إخراج الدواء من عبوته إلا قبل إعطائه مباشرة.
يجب إعطاء الدواء بحسب الجدول الزمني المقرر وتدوين ذلك في سجل إعطاء الدواء.

التحدث مع المريض أو من يقوم بالاهتمام به عن اسم الدواء والهدف من تناوله ومفعوله وخصوصاً عند إعطائه لأول مرة، وتكرار هذا الأمر في المرات اللاحقة.

عدم استعارة الدواء من مريض آخر في حالة فقدان الجرعة الخاصة بالمريض.
يجب القيام بتدريب العاملين في التمريض على الأجهزة الخاصة بإعطاء الدواء، والقيام بالمراقبة الدورية أو التأكد من كفاءة مستعمليها من حيث تشغيلها ومعرفة القيود الخاصة بها.

يجب القيام بتقييم كفاءة العاملين في التمريض من ناحية المعلومات الدوائية في بداية العمل ثم بشكل مستمر.

الاستماع لشكوى المريض عند شعوره بحدوث أثر جانبي للعلاج أو الدواء.
الاستماع لاستفسارات المريض أو عند رفضه تناول الدواء ، والتحقق مرة ثانية من الأمر الطبي الأصلي.

يجب فحص العوامل المحيطية التي تؤثر على القيام بالعمل في أجنحة التمريض، مثل الإضاءة، والضجيج، ودرجة الحرارة، وتكرار أسباب التشتيت عن العمل ( الهاتف، الأشخاص، القيام بمهمات لا علاقة لها بالعمل الأصلي... الخ).
يجب توفير العدد الكافي من العاملين والمصادر الأخرى التي تساعد على تحمل ضغط العمل.

استعمال النظم الآلية (المؤتمتة) كالنظام المباشر لإدخال الأوامر الطبية، أو سجلات إعطاء الدواء المبرمجة على الحاسب، أو الشرائح المشفرة Bar Coding، وذلك لتسهيل مراجعة الوصفات وزيادة دقة إعطاء الدواء والإقلال من الأخطاء التي تقع أثناء نسخ الوصفات الطبية.






الوصفة الطبية: هي الوصفة التي يكتبها الطبيب للمريض الخارجي (غير المنوم في المستشفى) والتي تُصرف عادة من الصيدلية الخارجية.

الأمر الطبي: هي وصفة يكتبها الطبيب فيها كل ما يتعلق برعاية المريض المنوم بما في ذلك العلاج الدوائي.

الصيدلة الإكلينيكية ( السريرية)



يمكن تعريف الصيدلة الأكلينيكية على أنها: مجال من مجالات ممارسة مهنة الصيدلة في الهيئة الصحية، حيث يقوم الصيدلي بعمله كعضو في الفريق الطبي الذي يُعنى بالمريض ، فيقوم بتطبيق رؤيته السريرية (الأكلينيكية) لضمان الاستخدام الصحيح والآمن للأدوية.
إن هذا الفرع من مهنة الصيدلة يتطلب دراسات متخصصة في مناهج الصيدلة وتدريباً مُنظماً، لكي يتمكن الطلاب من الوصول إلى الهدف المهني المطلوب.

وبناءً على التعريف السابق، فإن الصيدلي الأكلينيكي، هو الصيدلي الذي يقوم بأداء عمله في أجنحة المرضى، حيث يشارك في المرور على المرضى كعضو في الفريق الطبي،
ويقوم بالوظائف التالية

1- الحصول على تاريخ المريض العلاجي.

2- إختيار الوسائل العلاجية الأكثر ملائمة ومراقبة سير العلاج.

3- الإجابة على استفسارات الأطباء وغيرهم من العاملين في السلك الصحي عن المعلوات المتعلقة بالأدوية.

4- تعليم المرضى والرد على استشاراتهم.

5- توفير التعليم المتعلق بالدواء داخل نطاق الخدمة الصحية للأطباء واسلك التمريض وغيرهم من العاملين في الحقل الصحي.

ولقد أُدخلتْ في حقل الصيدلة الأكلينيكية وبشكل خاص ،إذا صح القول، وظائف أخرى إضافة إلى ما ذُكر أعلاه مثل :

1- حركية الدواء الأكلينيكية (السريرية).

2- خدمات التغذية الوريدية.

3- ممارسة علم السموم الأكلينيكي تحت مسمى مركز المعلومات المتعلقة بالسموم.

4- التحريات الدوائية الأكلينيكية.

5- أستشارات المعالجة الدوائية الأساسية.

6- المشاركة في تقييم استعمال الدواء.

7- مزاولة الصيدلة الأكلينيكية المتخصصة في مختلف الحقول الطبية ، مثل الأورام أو الكلية أو الجراحة او الأطفال أو العناية المركزة وغيرها. إنّ هذه الاختصاصات تتطلب تدريب متخصص يتم من خلال ما يسمى برنامج الإقامة (يدعى الصيدلي خلالها صيدلي أكلينيكي مقيم) وغيرها من البرامج.
