خافضات الضغط



يعاني كثير من المرضى من ارتفاع ضغط الدم، ويستخدم الكثير منهم أكثر من نوع للتحكم في مستوى ضغط الدم، أشهر الأنواع المستخدمة في التحكم في ضغط الدم الشرياني .

هناك خمس مجموعات رئيسة من أدوية علاج ضغط الدم المرتفع، ويعتمد اختيار علاج المريض على الحالة الصحية وعلى موانع استخدام الدواء لكل مريض، وهذه الأنواع هي:




1- مدرات البول: أوسع الأدوية انتشاراً، وأرخصها ثمناً، وأكثرها فاعلية. وتعمل هذه الأدوية على منع امتصاص الصوديوم (الموجود في ملح الطعام) من القنوات الكلوية مما يؤدي إلى إخراج الصوديوم والماء بكميات زائدة. ويندرج تحت هذه المجموعة ثلاث مجموعات رئيسة:



أ - مجموعة الثيازايد:Thiazide

أكثر الأدوية استعمالاً وهي ذات كفاءة عالية إذا لم يكن هناك

ضعف في وظائف الكلى. تعطى هذه العلاجات بالأخص لكبار السن، ويمكن استخدامها بأمان لمرضى السكري ولمن لديهم ارتفاع في دهون الدم. يمنع استخدامها لمن لديه حساسية أو ارتفاع في نسبة البوليك (مرض النقرس).

الكورتيكوستيرويد أوالهيدرو كورتيزون تزيد من نسبة البوتاسيوم في الدم لذا لاتعطى مع الثيازايد



ب - المجموعة المؤثرة على أنبوب هنلي: Loop(مثل عقار لاسكس) وهي أشد المجموعات تأثيراً حتى مع ضعف وظائف الكلى، وتأثيراتها على السكري والدهون وحمض البوليك أقل من المجموعة السابقة إلا أنها يمكن أن تسبب انخفاضاً شديداً في نسبة البوتاسيوم خاصة مع الجرعات الكبيرة.

2- مغلقات مستقبلات بيتا: هذه المجموعة أيضاً من أوسع الأدوية انتشاراً وأكثرها فائدة، تزداد فائدة هذه المجموعة بشكل خاص في مرضى ارتفاع الضغط المصاحب لقصور الشرايين التاجية أو اضطرابات النبض. وأما الأعراض الجانبية الرئيسة لها فهي ضيق الشعب أو أزمة الربو، وكذلك في أحيان قليلة تعسر التوصيل الكهربي للقلب. يعاني بعض المرضى المستخدمين لها من الإرهاق عند بذل المجهود، كما يعاني نحو 10% من الذكور من ضعف الانتصاب. وقليلا ما يحدث اضطرابات النوم أو الاكتئاب. ينصح المريض بقدر الإمكان بعدم التوقف عن استعمال هذه الأدوية بطريقة مفاجئة.

Side Effects and Contraindications

bradycardia, reduced exercise capacity, heart failure, hypotension, and atrioventicular (AV) nodal conduction block

a beta-blocker is given in conjunction with cardiac selective calcium-channel blockers (e.g., verapamil) because of their additive effects in producing electrical and mechanical depression

contraindicated in heart failure patients

contraindicated in patients with sinus bradycardia and partial AV block.


Bronchoconstriction can occur, especially when non-selective beta-blockers are administered to asthmatic patients. Therefore, non-selective beta-blockers are contraindicated in patients with asthma or chronic obstructive pulmonary disease. Bronchoconstriction occurs because sympathetic nerves innervating the bronchioles normally activate b2-receptors that promote bronchodilation. Blockade of these receptors can lead to bronchoconstriction.
Hypoglycemia can occur with beta-blockade because b2-adrenoceptors normally stimulate hepatic glycogen breakdown (glycogenolysis) and pancreatic release of glucagon, which work together to increase plasma glucose. Therefore, blocking b2-adrenoceptors lowers plasma glucose. b1-blockers have fewer metabolic side effects in diabetic patients; however, the tachycardia which serves as a warning sign for insulin-induced hypoglycemia may be masked. Therefore, beta-blockers are to be used cautiously in diabetics.


3- مغلقات مستقبلات أيون الكالسيوم:يمكن استعمال هذه الأدوية في المرضى من كبار السن، وكذلك إذا كان ارتفاع ضغط الدم مصحوباً بقصور الشرايين التاجية أو ارتفاع دهون الدم أو السكر. هذه الأدوية لا تحدث ضيق الشعب الهوائية أو الإرهاق وضعف الانتصاب الذي قد يحدث مع المجموعة السابقة. الأعراض الجانبية الرئيسية هي تورم القدمين والصداع.

There are three classes of CCBs

the dihydropyridines

generally used to treat angina

Non-dihydropyridines
Verapamil (phenylalkylamine class), is relatively selective for the myocardium, and is less effective as a systemic vasodilator drug. This drug has a very important role in treating angina (by reducing myocardial oxygen demand and reversing coronary vasospasm) and arrhythmias. Diltiazem (benzothiazepine class) is intermediate between verapamil and dihydropyridines in its selectivity for vascular calcium channels. By having both cardiac depressant and vasodilator actions, diltiazem is able to reduce arterial pressure without producing the same degree of reflex cardiac stimulation caused by dihydropyridines.

Long-acting dihydropyridines have been shown to be safer anti-hypertensive drugs, in part, because of reduced reflex responses. The cardiac selective, non-dihydropyridine CCBs can cause excessive bradycardia, impaired electrical conduction (e.g., atrioventricular nodal block), and depressed contractility. Therefore, patients having preexistent bradycardia, conduction defects, or heart failure caused by systolic dysfunction should not be given CCBs, especially the cardiac selective, non-dihydropyridines. CCBs, especially non-dihydropyridines, should not be administered to patients being treated with a beta-blocker because beta-blockers also depress cardiac electrical and mechanical activity and therefore the addition of a CCB augments the effects of beta-blockade.

4- مغلقات مستقبلات ألفا: ليس لهذه المجموعة تأثير سلبي على دهون الدم أو نسبة السكر في الدم، وبالإضافة إلى ذلك فإن لها خاصية إرخاء عنق المثانة البولية، لذلك فهي تستخدم لخفض ضغط الدم لدى الذكور ممن يعانون تضخم البروستاتا مع صعوبة التبول.

5- مثبطات الأنزيم المحول للأنجيوتنسين: ازداد استعمال هذه الأدوية كثيراً خلال السنوات العشرين الماضية. تعمل هذه الأدوية على منع تكوين انجيوتنسين 2القابض لجدران الأوعية الدموية، وكذلك على منع تكسير براديكينين الموسع للأوعية الدموية. بالإضافة إلى ذلك فإن لهذه الأدوية خواص مضادة لتجلط الدم وخواص مانعة لتصلب الشرايين.
وهي أكثر تأثيراً لدى مرضى السكري حيث تكون لديها القدرة على منع أو تأخير التلف الذي يحدثه السكر بالكليتين والإقلال من حدوث النوبة القلبية. ليس لهذه الأدوية أي آثار ضارة على الدهون أو مستوى السكر في الدم أو حمض البوليك،
ولكن قد يحدث أحياناً ارتفاع في نسبة البوتاسيوم أو تأثر في وظيفة الكليتين. أما أكثر أعراضها الجانبية انتشاراً فهو السعال الجاف، الذي يصيب نحو 10% من المرضى وفي بعض الأحيان يكون من الشدة بحيث يوجب التوقف عن استعمال الدواء
(وهناك مجموعة مشابهة هي مثبطات مستقبلات الانجيوتنسين2،
وهذه الأدوية لها تقريباً نفس التأثيرات العلاجية لمثبطات الأنزيم المحول للأنجيوتنسين إلا أنها لا تحدث السعال الجاف)
يتم استخدام هذه الأدوية منفردة أو مع مدرات البول في قرص واحد.
يمنع تماماً استخدام هذه الأدوية على الحوامل.

الطريقة السليمة لإستعمال الأدوية




وكذلك يجب إخباره إذا كنت مريض بأحد الأمراض المزمنة مثل السكر أو كنت تأخذ حمية خاصة. لان اكثر الأدوية تحتوي على خليط من المكونات يكون المختص يعرف أثار استعمالها مع بعض. ويجب على النساء إخبار الطبيب أو الصيدلي قبل استعمال الدواء بوجود الحمل أو بنية الحمل خلال فترة استعمال الدواء لان بعض الأدوية تسبب عيوب خلقية للوليد أو مشاكل أخرى خلال الحمل إن استعمال أي نوع من الأدوية خلال الحمل بدون استشارة طبية يسبب بعض المشاكل لذا يجب الامتناع من الاستماع لنصائح غير المختصين باستعمال أي دواء خلال فترة الحمل.

أما ألام المرضع يجب أن تعلم إن بعض الأدوية ينتقل تأثيرها إلى الطفل من خلال الحليب وقد تسبب ثاثيرات غير مرغوبة في الرضيع لذا يجب إخبار الطبيب أو الصيدلي بان ألام في فترة الرضاعة.

إذا كنت تأخذ من بعض الأدوية التي تصرف بدون وصفة طبية (OTC) مثل مسكنات الألم أو معدلات الحموضة وغيرها لا تنسى إخبار الطبيب والصيدلي بذلك مع قراءة النشرة المرفقة بعناية قبل الاستعمال.

تخزين الأدوية

من المهم أَنْ تقوم بتخزين أدويتكَ بشكل صحيح. ودليل التخزين الصحيح يتضمن :

أحفظ ألادوية بعيداً عن متناول الأطفال.

احفظ الأدوية في علبها الأصلية.

احفظ الأدوية بعيدا عن الحرارة والضوء المباشر.

لا تحتفظ بالأدوية المحتوية على كبسولات أو أقراص بالحمام أو قرب مغسلة المطبخ أو في الأماكن الرطبة الأخرى لأنها تسبب تلف وتحلل الأدوية .

اغلق علب الأدوية بأحكام وتخلص من القطن الموجود في بعض العلب لأنه قد يساعد على الرطوبة بعد فتح العلبة للاستعمال.

لا تَخْزنُ الأدوية في الثّلاجةِ ما لم يخبرك الصيدلي بضرورة ذلك .

لاتحفظ الأدوية السائلة في درجة التجمد داخل (الفريزر)

لا تَتْركُ أدويتكَ في السيارةِ لفترات طويلة من الوقت .

لا تَحتْفظُ بالأدوية القديمة وتخلص منها بمكان لا يصل له الأطفال .

كيفية استعمال الادوية بصورة صحيحه :

تقسم الأدوية إلى عدة أصناف تؤخذ بواسطة:


الفم الجلد

الأنف العين

الأذن الشرج

المهبل


لذا يجب استعمال الدواء حسب التعليمات المعطاة لك حسب المواعيد الدقيقة والجرعة الموصوفة لك والمدة المحددة من قبل الأخصائيين وإذا شعرت بان الدواء الذي استعملته لم يأتي بنتيجة أو تسبب لك ببعض الأعراض الجانبية اتصل بطبيبك واخبره بذلك.

إذا لم يقم الصيدلي بمزج الأدوية مع بعض لا تقم بذلك واحفظ كل دواء بعلبته الأصلية واحكم إغلاق الغطاء بعد الاستعمال ولا تنزع تعليمات الاستعمال من العلبة لأنك قد تحتاج لقراءتها من وقت لأخر.

لتجنب الأخطاء غير المقصودة لاتاخذ الدواء بالظلام وانتبه إلى تاريخ صلاحية الدواء.


الأدوية التي تؤخذ بواسطة الفم :
بشكل عام، إنه من الأفضل أَنْ تؤخذ الأدوية مع كاس كامل من الماء ولكن يجب الانتباه إلى الطبيب أو الصيدلي عند إعطاء تعليمات الاستعمال لان بعض الأدوية يجب استعمالها مع الطعام وبعضها حين تكون المعدة فارغة.

لأتكسر كبسولة الدواء أو تمضغها إلا إذا طلب منك ذلك . إذا كنت تستعمل أدوية سائلة يجب استعمال ملعقة فيها مقاس أو أي أداة أخرى تقيس الجرعة بدقة يجب عدم استعمال ملاعق الطعام لأنها لا تعطي المقاسات الدقيقة أسأل الصيدلَي حول هذه الأدوات .

الأدوية التي تؤخذ بواسطة الفم تأتي على شكل حبوب أو كبسولات أو سوائل إذا كانت لديك مشكلة في ابتلاع الجرعة اتصل بطبيبك واخبره بالمشكلة حتى يجد لك البديل المناسب الذي ممكن أن يكون اكثر سهوله في ابتلاعه.

الأدوية التي تؤخذ بواسطة الجلد:

إذا كانت على شكل لصقه يجب أن يكون الجلد جاف ونظيف وخالي من الشعر والندوب .

أزل الرقعة السابقة قبل وضع اللصقه الجديدة.

أدوية الأنف:
أدوية الاستنشاق عادة تاتي مع تعليمات واضحة للمريض، أقرا التعليمات بحذر قبل استعمال الدواء. بما إن هنالك أدوية استنشاق مختلفة ويتم استخدامها بعدة طرق فمن المهم جدا قراءة التعليمات المرفقة بعناية إذا لم تفهم التعليمات أو لم تعرف كيف تستعمل الدواء أتصل بالاخصائي .

قطرات الأنف
استعمالها يكون بإمالة الرأس بينما باقي جسمك يكون منتصب أو تنام على ظهرك على السرير ويكون رأسك على الجانب ثم ضع القطرات بدون عصر القطرة وبلطف في كل جانب من الأنف واحتفظ بنفس الوضع لبضعة دقائق حتى يدخل الدواء إلى داخل الأنف ثم اشطف القطارة بالماء الدافئ وامسحها بقطعة نظيفة واحكم إغلاقها وحتى تتجنب التلوث لا تستعمل نفس قطرة الأنف لا كثر من شخص .

بخاخ الأنف
طريقة استعمال بخاخ الأنف اضغط علبة البخاخ في كل جانب بشكل سريع وقوي .

بعد الاستعمال أشطف رأسَ علبة البخاخ بالماءِ الحارِ، ولكن أحذر من دخول الماء إلى داخل العلبة ثم جففها بمنديل نظيف واغلق العلبة بأحكام ولا تستعمل نفس علبة البخاخ لأكثر من شخص حتى لا تنتقل العدوى

قطرات العين:
لَمْنعَ التلوث، لا تَتْركُ رأس قطرةِ العين يلمس أي سطحِ (من ضمنها العين) ويجب حْفظُ العلبة مغلقة بأحكام.

القطارة يجب أن لأتكون ممتلئة حتى يتمَ السيطرة على القطرةَ بصورة صحيحةَ.

كيفية الاستخدام: أولا، أغسل يديٌكَ. أرجع رأسكَ إلى الخلف وبالسّبابةِ، أسحب الجفن الأسفل إلى تحت حتى يعطي شكّلَ الجراب. ضع الدواء في جرابِ العين وأغلق العين برفق. لا تَغْمزُ. أجعل عيونكَ مغلقة لمدة دقيقة إِلى دقيقتينِ.

إذا كانت أدويتك خاصة باللجلوكوماِ أو التهاب العينِ: أتبع التعليمات الموضحة أعلاه، على أية حال، بعد وَضْعِ القطراتِ في عينكَ، أضغطَ زاويةِ العينِ بإصبعكَ الأوسط مباشرة. أستمر فيْ الضغط لمدة دقيقة إِلى دقيقتين بعد وضع الدواء في العينِ. هذا سَيُساعدُ بمْنع الدواء من أن يتفاعل في الجسمِ ويتسبب في أثار جانبية.

بعد وضع قطرات العين أغسل يديك لإزالة أي دواء عليها.

مراهم العين:
حتى تَمْنعَ تلوث مرهمِ العينِ، لا تَتْركُ راس أنبوب المرهم تلمس أي سطح ( بضمن ذلك العين ). بعد الاستعمال،امسح رأسِ أنبوب المرهمِ بقطعة نظيفِ واَحْفظُ الأنبوب مغلق بأحكام.

كيفية الاستخدام : قبل الاستعمال اغسل يديك جيدا اسحب الجفن السفلي على شكل جراب ثم اعصر شريط رقيق من المرهم في الجراب اغلق عينيك برفق لمدة دقيقة إلى دقيقتان ثم اغسل يديك ولا تترك أي اثر للمرهم على يديك.

قطرات الأذن:
لمنع التلوث يجب أن لأتلامس القطارة الجلد ( بضمن ذلك الأذن ). وكذلك يجب عدم ملئ القطارة حتى تتمكن من السيطرة عليهاَ. عند إعطاء القطرة يجب على المريض أن ينام على الفراش على جنب وبلطف اضغط على القطارة ليدخل الدواء إلى داخل قناة الأذن استمر بنفس الوضع لمدة من 3- 5 دقائق حتى يصل الدواء إلى قاع قناة الأذن .

لا تغسل القطارة بعد الاستعمال إنما امسحها بمنديل نظيف وأغلقها بأحكام .

تحاميل الشرج:


أولا اغسل يديك جيدا ثم أزل الغلاف القصديري للتحميلة ورطب التحميلة بالماء نم على جانبك ادفع التحميلة في الشرج بواسطة اصبعك إذا كانت التحميلة غير متماسكة اتركها قبل استعمالها وقبل إزالة غلافها في الثلاجة أو داخل ماء بارد لمدة 30 دقيقة اغسل يديك جيدا بعد إدخال التحميلة.

الادوية المهبلية :
إذا كنت حامل يجب أن لا تستعملي أي أدوية توضع داخل المهبل إلا بأشراف طبي.

أما إذا كان للعلاج أولا اغسلي يديك جيدا ثم أدخلي العلاج بلطف وبدون استعمال أي قوة داخل المهبل انتظري بِضْع دقائقَ قبل النُّهُوضِ.

اغسلي يديكَ بالصّابونِ والماءِ الدّافئِ.

تحذيرات واحتياطات وقائية
لا تعطي من أدويتك لأي شخص آخر فالدواء وصف لحالتك أنت فقط ويحتمل أن يكون ضار للشخص الآخر .

العديد من الأدوية لا يجب أن تؤخذ مع أدوية أخرى أو المشروبات الكحولية لذلك عليك سؤال الطبيب والصيدلي عن هذه المسالة.

قبل جراء أي عمليات جراحية بما فيها عمليات جراحة الأسنان يجب إخبار الطبيب عن أنواع الأدوية التي تستعملها .

إذا اعتقدت انك أخذت جرعة زائدة من الدواء أو أحد الأطفال اخذ الدواء بطريقة خاطئة اتصل بالإسعاف فورا (من المفروض انك تعرف رقم هاتف الإسعاف ) ومن الضروري أن يكون لديك زجاجة من شراب يساعد على التقيؤ (متوفر في الصيدليات ) والذي قد يطلب منك استعمالها ولكن يجب حفظها بمكان آمن بعيد عن الأطفال وقراءة تعليمات استعمالها بدقة.

التأثيرات الجانبية
إن استعمال الأدوية قد يسبب بعض التأثيرات الجانبية غير المرغوبة وبعض هذه التأثيرات ربما تحتاج إلى انتباه ورعاية طبيه لذا عليك سؤال الطبيب أو الصيدلي عن التأثيرات الطبية المتوقعة من استعمال الدواء وفي حالة حصول ردود فعل غير اعتيادية يجب الاتصال بالطبيب على الفور أو مراجعة الإسعاف .

معلومات إضافية
إنها فكرة جيدة لك أَنْ تعود نفسك بان تعرف الاسم العلمي والاسم التجاري للدواء َالذي تريد أن تستعمله ومن الأفضل أن تقوم بكتابته من اجل الاستعمال المستقبلي .

إذا كنت تستعمل نفس الدواء بصورة دائمة لا تنتظر إلى أن تنتهي علبة الدواء لطلب علبة جديدة حيث انه من الممكن انك لن تستطيع أن تحصل على هذا الدواء بسبب نفاذه من الصيدلية أو انه انتهى لديك في وقت غير مناسب.

عندما تُسافر احمل كمية كافية من أدويتك معك ولا تضعها مع العفش حيث يمكن أن يفقد أو يتعرض إلى درجات حرارة باردة أو حارة وتأكد بان تأخذ نسخة من وصفتك حيث إن بعض الدول لأتسمح بدخول بعض الأدوية أليها .

أخيرا لآتكن محرجا من السؤال عن أي دواء تستعمله سواء إلى الطبيب أو الصيدلي .

المقشعات أدوية تزيد افراز القشع وتميعه وتسهل بالتالي خروجه من القصبات الكبيرة والصغيرة والرئتين
وبذا تحرر المجاري التنفسية من المفرزات المعيقة لعملية التنفس والمثيرة للسعال
ولذا فهي تستعمل في علاج التهاب القصبات bronchitis

التي تزيد من افراز القشع وبذا تحمي الاغشية المخاطية المخرشة
وتخفض نوبات السعال

تؤدي إلى إفراز القشع والى سعال منتجproductive cough
ويقل حدوث نوبات السعال يضم هذا القسم

Overview of pharmaceutical excipients used in tablets and capsules




The pharmaceutical industry is ever thirsty to satisfy patient’s therapeutical needs and apart from active ingredients, inactive excipients play a major role in formulation development. Pharmaceutical excipients are substances other than the pharmacologically active drug or prodrug which are included in the manufacturing process or are contained in a finished pharmaceutical product dosage form.

In addition to transporting the active drug to the site in the body where the drug is intended to exert its action, excipients play an important part in the manufacturing process. They may also be important for keeping the drug from being released too early in the assimilation process in places where it could damage tender tissue and create gastric irritation or stomach upset.

Others help the drug to disintegrate into particles small enough to reach the blood stream more quickly and still others protect the product's stability so it will be at maximum effectiveness at time of use. In addition, some excipients are used to aid the identification of a drug product.

Last, but not least, some excipients are used simply to make the product taste and look better. This improves patient compliance, especially in children. Although technically "inactive" from a therapeutic sense, pharmaceutical excipients are critical and essential components of a modern drug product. In many products, excipients make up the bulk of the total dosage form (1). Apart from the drug’s active ingredient, other essential components include diluents or fillers, binders, disintegrants, lubricants, coloring agents and preservatives (3). Diluents or fillers are inert ingredients that can significantly affect the chemical and physical properties of the final tablet thus affecting the biopharmaceutical profile.

One classic example of this are calcium salts, which can be utilized as fillers, which interfere with the absorption of tetracycline from the gastrointestinal tract. (4) This example emphasizes that excipients may not always be inert, as they may be perceived.

Usually tablets are designed so that the smallest tablet size which can be conveniently compressed is formed. Thus, if the dose is small more diluents are required and if the dose is high less diluents are required as not to increase the tablet size, which might make it difficult to swallow.

Diluents selection should be made carefully as physical-chemical changes might render the product unstable and might cause problems in manufacturing. Binders are added to tablet formulations to add cohesiveness to powders thereby providing the necessary bonding to form granules which under compaction form a compact mass as tablet. In other words, binders are essential to achieve the “hardness” of the tablet.

Binders are usually selected on basis of previous experience, particular product needs, literature or vendor data or the preference of individual scientists or manufacturing unit. The primary criterion when choosing a binder is its compatibility with other tablet components.

It must add sufficient cohesion to the powders to allow for normal processing yet allow the tablet to disintegrate and the drug to dissolve upon ingestion, releasing the active ingredients for absorption. Disintegrants facilitate the breakup of a tablet after oral administration.

They can be added prior to granulation or during the lubrication step prior to compression or at both processing steps. The effectiveness of many disintegrants is affected by their position within the tablet. Since disintegration is the opposite operation to granulation (agglomeration) and the subsequent formation of strong compacts, one must carefully weigh these two phenomena when designing a tablet. Lubricants prevent sticking of the tablets to the tablet punches during the compression phase of the tablet manufacturing process.

When lubricants are added to a powder mass, they form a coat around individual particles which remains more or less intact during compression. Lubricants are mostly hydrophobic. The presence of lubricant coating may cause an increase in the disintegration time and a decrease in drug dissolution rate. The choice of a lubricant may depend upon the type of tablet being manufactured, dissolution, flow characteristics and requirements of the formulation in terms of hardness, friability and compatibility. Glidants are the materials that have good flow properties and poor lubrication properties.

Glidants improve the flow of powder into the tableting machines for compaction. They act to minimize the tendency of a granulation to separate or segregate due to excessive vibration. High speed tablet machine require smooth even flow of material to die cavities (tablet mold). The uniformity of tablet weights directly depends on how uniformly the die cavity is filled. In general many materials commonly referred to as lubricants possess only a minimal lubricating activity and are better glidants or anti-adherents.

Thus a blend of two or more materials may be necessary to obtain these properties. Pharmacists should be familiar with the components of pharmaceuticals products, beyond their active ingredients. In order to educate pharmacists on excipients that are routinely used in the pharmaceutical industry, we decided to examine the top 200 prescription tablets and capsules products of 2003 (2) and find out how many or which excipients are used in each product.

The selection will cover both brand and generic drugs. Out of the 200 prescription drugs, the total numbers of inactive excipients used except for coating and coloring agents were ONLY 94! Although the list is composed on the top 200 drugs of 2003, very few blockbusters has been launched since then and still the excipients in all remains the same.

From the table above we see that most of the commonly used ingredients have not been changed in making a tablet from generations and might follow the same trends for many generations to come.

CONCLUSION: Lot of time and effort are still needed in field of excipients. However till then a formulation scientist is entrusted with the limited amount of excipients and will have to deal with 94 excipients or now maybe 104.

Abiotic Degradation

Carcinogen
A compound which is capable of causing cancer.

Carcinogenic
The ability of a substance to cause cancer.

Carcinogenicity
The complex process whereby normal body cells are transformed to cancer cells.

Case-Control Study
A type of epidemiology study to investigate the cause of the toxicity. It compares the exposure histories of humans who have a particular toxic effect with that of normal individuals.

Ceiling Level
The maximum allowable concentration of a chemical in the workplace for a specific period of time (usually 15 minutes).

Cell Transformation
The change of a cell from one form to another. The term is generally used to denote the change from normal to malignant.

Cholinergic Effect
Neurological effects resulting from the transmission by acetylcholine of impulses across synaptic junctions between nerves.

Cholestasis
A liver condition in which excretion of bile salts via the bile duct is inhibited resulting in bile salts backing up into liver cells.

Chromatography
An analytic method to separate and identify the components of a complex mixture by the differential movement through a two-phase system. The movement is effected by a flow of a liquid or a gas (mobile) phase, based on the physicochemical principles of adsorption, partition, ion exchange, exclusion, or a combination of these principles.

Chromosome
One of a group of structures that form in the nucleus of a cell during cell division. Chromosomes, bearing the DNA, carry the genetic code for the organism.

Chromosome Aberration
Changes in chromosome structure.

Chronic Dose
Substance administered or received gradually over a long period of time (months to years).

Chronic Effect

Cirrhosis

CNS

Conjugation

Control Group
A group of animals or humans in a study that are treated the same as the exposed groups but without receiving the specific exposure.

Cornea
The transparent front surface of the eye.

Corrosion
Direct chemical action that results in irreversible damage at the site of contact. It is manifested by ulceration, necrosis, and scar formation.

Covalent Bond
The joining together of atoms that results from sharing electrons.

CPSC
Consumer Product Safety Commission. It is a federal agency responsible for protecting the public from toxins and other hazards present in consumer products.

Cross-Sectional Study

Dosage
The determination of quantity of a substance received that incorporates the size, frequency, and duration of doses (e.g., 10 mg every 8 hours for 5 days).

Dose
The amount of a substance received at one time. Dose is usually expressed as administered or absorbed dose (e.g., milligrams material/kilogram of body weight).

Dose-Response Assessment
The relation between dose levels and associated effects.

Dose-Response Curve
A graphical representation of the quantitative relationship between doses of a substance and specific biological effects.

Edema

Effluent

Embryo

Embryotoxic
The harmful effects of a substance on the developing embryo.

Environmental Fate
The fate of a substance following its release into the environment. It includes the movement and persistence of the substance.

المطهرات

المطهرات الكيميائية مواد تبطئ أو توقف نمو الكائنات المجهرية (الجراثيم) على الأسطح الخارجية من الجسم وتساعد على منع العدوى. وينبغي التمييز بين المطهرات من المضادات الحيوية التي تدمر الكائنات الحية الدقيقة داخل الجسم ، ومن المطهرات التي تدمر الكائنات الحية الدقيقة الموجودة في الكائنات (غير الحية) الجماد. ومع ذلك ، غالبا ما يشار إليها باسم المطهرات و المطهرات الجلدية

ويمكن استخدام معظم العوامل الكيميائية ، على حد سواء من المطهرات .و يتم تحديد الغرض الذي يتم استخدامه من أجله حسب تركيزه. على سبيل المثال بيروكسيد الهيدروجين يستخدم الحل 6 ٪ للجروح والتطهير ، في حين يتم استخدام أقوى الحلول (> 30 ٪) في قطاع الصناعة باعتباره عامل مؤكسد للتبييض.

أنواع المطهرات




Alcohols الكحول
تمثل الكحول طيفاً واسعاً للقضاء على البكتيريا.
إن البكتيريا تقتل عندما تتعرض لتركيز عالي من الكحول 70%، إن تركيز الكحول مهم جداً، حيث أنه لو زاد التركيز يحدث جفاف لدى الخلية الجرثومية معطياً إياها القدرة على مقاومة الكحول.
ونتيجة لذلك تبقى الجراثيم المعرضة للكحول لفترة أطول.

مركبات الأمونيوم الرباعية

استخدامات المطهرات





وهي تستخدم أساسا للحد من مستويات الكائنات الدقيقة على الجلد والأغشية المخاطية. الجلد والأغشية المخاطية في الفم والأنف والمهبل تعد موطنا لعدد كبير من الكائنات الحية الدقيقة وعادة ما تكون غير مؤذية . الا عند حدوث تلف في الأغشية المخاطية أو الجلد أو خرق في الجراحة ، ويمكن استخدام المطهرات لتطهير المنطقة المصابة و هي تقلل من فرص العدوى.و من المهم أيضا أن الناس الذين يعالجون المرضى الذين يعانون من الجروح أو الحروق غسل أيديهم بشكل كاف عند حل المطهر واستخدامه لتقليل خطر العدوى.






غلوكونات الكلورهيكسيدين والبوفيدون -- غسل اليدين وغالبا ما يستخدم المحلول في الدعك باليد و التدليك باليد .


قبل العملية تطهير الجلد -- المطهرات تطبيقها على موقع العملية للحد من فلورا الجلد


في مجرى البول والمثانة أو المهبل لعلاج الالتهابات عن طريق الري أو يطهر التجويف قبل القسطرة -- تطهير الغشاء المخاطي.


منع وعلاج الجروح والحروق المصابة --

علاج التهابات الفم والحنجرة مثل

-- له خصائص مضادة للجراثيم والفطريات وهو العنصر النشط في معينات الحلق














الاحتياطات






المطهرات التي تشترى من الصيدلية يجب أن لا تستخدم لأكثر من أسبوع واحد. إذا كانت المنطقة المصابة لم تلتئم أو تتحسن في هذا الوقت يجب عليك التوقف عن استخدام المطهر وراجع طبيبك.





الجروح الكبيرة والجروح البالغة والحروق أكبر من بقعة حمراء صغيرة ، وجروح مع جزيئات راسخة لا تغسل ، ولدغ الحيوانات وإصابات العين يجب ان تعامل من قبل الطبيب.



لا تستخدم المطهرات لعلاج حروق الشمس أو الالتهابات الجلدية الموجودة. تذكر أن المطهرات فقط تقلل الكائنات الدقيقة الموجودة على سطح النسيج ، وسوف تكون هناك حاجة إلى استخدام المضادات الحيوية لعلاج العدوى داخل الأنسجة.




يجب على الناس الذين يعانون من الحساسية من أي نوع استشارة الطبيب أو الصيدلي قبل استخدام منتج مطهر على وصفة طبية. يمكن لبعض المطهرات تهيج البشرة وتسبب حساسية الجلد

عادات غذائية خاطئة
إبتعد عنها


1. تناول الطعام بسرعة


بما أننا نعيش في عصر السرعة، لا نعطي الوجبات الغذائية وقتها الكافي، فنأكل بسرعة لضيق الوقت. تشير الأبحاث أن تناول الوجبات الغذائية خلال فترة زمنية قصيرة وعدم مضغها جيدا يتسبب باستهلاك سعرات حرارية أعلى من المطلوب. وينصح بتخصيص وقت يتراوح بين 20 إلى 40 دقيقة لكل وجبة غذائية من أجل الاستمتاع بها.
ومن جانب آخر، أكد العلماء أن مضغ الطعام جيداً ولمدة أطول يعطي الجسم الوقت الكافي لتسجيل الشعور بالشبع بسرعة أكبر، مما يؤدي الى استهلاك أقل للسعرات الحرارية.



2. الاستغناء عن وجبة الفطور



من العادات الغذائية السيئة إهمال وجبة الفطور. تعتبر وجبة الفطور من أهم الوجبات حيث أنها توفر للجسم الحصول على المواد الغذائية بعد اتقطاع ساعات طويلة عن الطعام خلال فترة الليل. ويفقد الكثير من الناس شهيتهم لتناول وجبة الفطور بسبب شرب السوائل الساخنة مثل القهوة أو الشاي أو الحليب مباشرة عند نهوضهم من النوم، فيختفي شعورهم بالجوع، وسرعان ما يشعرون بالتعب والإعياء بوقت قصير بعد ذلك بسبب عدم تناول وجبة الفطور.
يفضّل أن تتكوّن وجبة الفطور من:

- أغذية غنيّة بالبروتين: بيض، أجبان 5% دسم، حليب 1% دسم.
- أغذية نشوية: خبز مصنوع من طحين قمح كامل
- خضار
- نوع من الدهون بكمية قليلة: زيت زيتون أو زيتون
وتعتبر وجبة الفطور في غاية الأهمية أيضا لطالب المدرسة حيث تشبر الأبحاث أن الأداء العقلي للطالب وقدرته على التركيز يرتفع عندما يتناول الطالب وجبة الفطور.



3. الإكثار من الأغذية المصنّعة

يرتبط استهلاك الأغذية المصّعة بالعديد من الاضطرابات الصحية والأمراض المزمنة. والمقصود بالأغذية المصنّعة هي تلك المنتجات الغذائية غير الموجودة في الطبيعة وإنما تنتجها الشركات الغذائية مثل البسكوت، الحلويات، النقرشات المالحة، الشوكولاتة، الحلاوة، الملبس، الخ.

تختلف الأغذية المصنّعة عن الأغذية الطبيعية في كونها أكثر كثافة بالسعرات الحرارية من الأغذية الموجودة في الطبيعة مثل الخضار، الفواكه، الحبوب، والبقوليات

الإيقاف المفاجئ للأدوية



الضرر الناجم عن الإيقاف المفاجئ للأدوية


يعتبر الإيقاف المفاجئ للأدوية من الأخطاء الشائعة التي يقع فيها الكثير من المرضى, وذلك لإحساسهم بتحسن الأعراض ,فيعتقد المريض انه لا حاجة لمتابعة الجرعة الموصوفة.


نشاهد هذه الظاهرة بصورة متكررة لدى استعمال المضادات الحيوية سواء عند الأطفال أو الكبار, و لعل هذا احد الأسباب الأساسية لازدياد المقاومة الجرثومية تجاه العديد من أصناف الصادات.




يترافق إيقاف الأدوية المفاجئ بنوعين من الأعراض:
1-متلازمة نكس الأعراض:

تمتاز بعودة الأعراض المرضية التي كان الدواء قد وصف لمعالجتها مما يشير إلى أن المعالجة الموصوفة لم تكن كافية للقضاء على المرض الأساسي و إنما ساعدت على إخفاء الأعراض مما جعل المرض يتفاقم و يعود للظهور بشكل أقوى.




2- متلازمة الحرمان:

و هي الحالة التي تحصل عقب إيقاف الأدوية المسببة للاعتياد, مثل الأدوية النفسية و المسكنات مركزية التأثير.




تزداد شدة الأعراض الناجمة عن التوقف الفجائي بتأثير بعض العوامل مثل شدة المرض و مقدار الدواء المستعمل , كما تتعلق أيضا بآلية استقلاب الدواء في الجسم ,فالدواء ذو الانطراح البطئ تتأخر أعراض انقطاعه عن الظهور.

إن الاستعمال المديد لبعض الأدوية يبدل التوازن العصبي في الجهاز العصبي الذاتي كما يبدل قيم الثوابت الكيماوية في النسج و الأخلاط ضمن الجسم و يؤثر في مستويات بعض الهرمونات و الإنزيمات
فكل إيقاف مفاجئ للدواء في هذه الحالة يحدث خللا في التوازن الجديد الذي تم الحصول عليه و استمر لأزمنة مختلفة ,و تزداد الأعراض شدة كلما كان تدخل الدواء اكبر في قيم ثوابت الجسم.


الوقاية:

تقع على عاتق الطبيب و الصيدلي, إذ عليهم توعية المريض حول خطأ بل و خطورة (أحيانا) الإيقاف المفاجئ للدواء و التأكيد كذلك على ضرورة أخذ الجرعة الموصوفة كاملة خاصة بالنسبة للصادات الحيوية التي يكتفي كثير من المرضى بتناول جزء منها (كما ذكرت بداية)

و لفهم ماهية الضرر الناجم عن إيقاف كل نوع منها لا بد من إيضاح بسيط لآلية عملها و سبب استعمالها



الكورتيكوستيروئيدات (مشتقات الكورتيزون)
هي مركبات صنعية أو نصف صنعية ذات طيف تأثير واسع جدا لذا تستعمل في الكثير من الحالات المرضية

أهم تأثيراتها:
1- اثر مضاد للالتهاب :لذا تستعمل في الأمراض الرثوية الحادة و المزمنة

2-اثر مضاد للتحسس: تعطى للسيطرة على الحالات التحسسية الحادة و المزمنة في مختلف أعضاء الجسم ( الربو القصبي\الحساسية الجلدية\التهابات العين و الأذن و الأنف التحسسية\
تستعمل في هذا السياق إما أشكال صيدلانية للتطبيق الموضعي(قطورات\مراهم\رذاذ استنشاقي\)
أو أشكال صيدلانية داخلية (حبوب\ شرابات\)
وتعطى حقنا للسيطرة على الأعراض الشديدة مثل حالة الصدمة التأقية سواء الدوائية أو المحدثة لسبب آخر.

3-كما توصف كجزء من العلاج في بعض الحالات الانتانية و التهابات الكبد.



إن هذه المركبات مشابهة لهرمون الكورتيزون الذي تفرزه الغدة الكظرية في الجسم ,و الذي يتدخل في معظم العمليات الحيوية في العضوية لذا فان لها آثارا فيزيولوجيةعديدة تختلف شدتها بين مركب و آخر


لكن ما هو الكورتيزون ؟




الكورتيزون عبارة عن هرمون تفرزه الغدة الكظرية الموجودة فوق الكلية و بالتحديد قشر الكظر.

. وهو ضروري جداً لوظائف الجسم مثل تنظيم كمية الأملاح في الجسم والسوائل في الجسم وضغط الدم ولون البشرة ومقاومة الالتهابات التحسسية وتنظيم إفرازات البروتين والإنزيمات,وتنظيم مستوى السكر في الدم وغيرها من الوظائف الحيوية والضرورية لوظائف الجسم وحيويته.

ونقص الكورتيزون يؤدي إلى اضطراب في الضغط والسكر والأملاح وهبوط مقاومة الجسم أثناء الصدمات والحوادث والالتهابات.

والجسم يفرز حوالي 25 ملغم من مادة الهيدروكورتيزون . (Hydrocortisone) خلال اليوم والليلة.


يسبب الإيقاف المفاجئ نوعين رئيسيين من الأعراض:\ لذا لا بد من إيقافها عبر إنقاص الجرعة تدريجيا\


1-نكس الأعراض:

إذ تعود الأعراض المرضية للظهور بشكل اشد ,وتتناسب هذه الشدة طرديا مع مقدار الجرعة المتناولة و مدة استعمالها, كما تلاحظ في الحالات التي لم تكن المعالجة بهذه الأدوية الخيار المناسب لها
مثال:تحول الربو الحاد أو الآفات التحسسية إلى الشكل المزمن
نشاط الآفات الرثوية بشكل شديد و إصابة المفاصل التي كانت سليمة أصلا قبل البدء بالعلاج



2- نقص مستوى هرمون الكورتيزون في الجسم

و ما يسببه ذلك من مخاطر عديدة
بسبب تثبيط الغدة الكظرية عند الاستعمال المديد لهذه الأدوية.
(تنطبق هذه الحالة على الأدوية المأخوذة داخليا)



كيف يتم إفراز الكورتيزون في الجسم؟

إن هنالك توازنا قائما بين الغدة النخامية الموجودة في قاع الجمجمة و قشر الكظر

النخامى تفرز هرمون ACTH الذي يحرض قشر الكظر على إفراز الكورتيزون, و عندما يصل هذا الأخير إلى مستواه الطبيعي في الجسم يحصل تثبيط تلقائي للنخامى بالية تسمى التلقين الراجع
وعندما يستهلك الجسم الكورتيزون أو يقل تركيزه في الدم تبدأ الغدة النخامية بإفراز ACTH وهكذا .


فعند إعطاء الكورتيكوستيروئيدات دوائيا و بكمية كبيرة و لمدة طويلة يحصل وفق الآلية المذكورة تثبيط للنخامى و نقص لإفراز هرمون ACTH و بالتالي يكون قشر الكظر في حالة راحة فإذا تم إيقاف العلاج فجأة سيصبح الجسم في حالة نقص شديد لهرمون الكورتيزون (و هي حالة خطيرة )


ذلك أن النخامى لا تستعيد عملها بصورة سريعة و مباشرة لذا نلجأ للإيقاف التدريجي للدواء ريثما تعود النخامى إلى أداء وظيفتها بصورة طبيعية من جديد.




مضادات التخثر الفموية : oral anticoagulants

إن تعرض الجهاز الوعائي لرض فيزيائي مثل جرح أو رض أو ثقب يفعل سلسلة معقدة من التفاعلات بين الصفيحات و الخلايا البطانية للأوعية الدموية و عناصر التخثر, و هذا يشكل علقة تغلق مكان الجرح و توقف النزيف.


أما تشكل الخثرات المرضي فهو يتم بنفس الخطوات التي يسلكها التخثر الفيزيولوجي ما عدا أن المحرض لها ليس عاملا خارجيا و إنما هو حالة مرضية في الجهاز الوعائي.



ما هي خطورة تشكل الخثرات؟
إن العلقة الملتصقة بجدار الوعاء الدموي تسمى خثرة بينما عندما تفقد تماسكها و تجول داخل الوعاء الدموي تسمى صمة و كل من الصمة و الخثرة خطر بسبب قدرتها على سد الوعاء الدموي ,

و هنا مكمن الأذى الذي تختلف طبيعته بحسب الوعاء الدموي الذي تعرض للانسداد
(مثل احتشاء القلب في حال انسداد الأوعية الإكليلية المغذية له,)



و بالتالي فان الأدوية المضادة للتخثر تقسم بحسب آلية تأثيرها إلى قسمين رئيسيين:

* مضادات تجمع الصفيحات الدموية مثل الأسبرين

*مضادات التخثر التي تتدخل في سير عملية تشكل الخثرة و تعرقلها
و هذه أيضا تقسم لمجموعتين رئيسيتين (تختلف بحسب آلية تأثيرها و استخدامها السريري):

الهيبارين\Hyparin\ و يعطى حقنا

مشتقات الكومارين \Coumarin\\\ مثل مركب \Warfarin\ و تعطى عن طريق الفم.


إن العديد من العوامل البروتينية التي تشترك في تفاعلات التخثر تعتمد على الفيتامين K كمساعد أنزيمي في عملها و مشتقات الكومارين هي مضادة للفيتامين K و بالتالي تثبط عملية التخثر .



تستعمل مانعات التخثر الفموية بشكل أساسي لمعالجة الخثار الوريدي العميق، وتستخدم أيضاً لمعالجة الانصمام الرئوي ومرضى الرجفان الأذيني ذوي الخطورة لحدوث الانصمام، وكذلك في حالة صمامات القلب الميكانيكية البديلة لمنع تشكل الخثرة على الصمامات.



يؤدي الإيقاف المفاجئ إلى متلازمة نكس الأعراض ,إذ يعود الميل بدرجة متفاوتة للتخثر ,مما قد يؤدي لحدوث خثرات ينجم عنها حوادث وخيمة \سواء على مستوى القلب أو الجملة العصبية\



لذا لا بد من إيقافها بشكل متدرج بإنقاص الجرعة تدريجيا و لمدة أسبوعين أو ثلاثة.







حاصرات المستقبلات B :Beta-Blockers


حاصرات B هي مجموعة من المركبات الدوائية ذات أثر حاجب للمستقبلات B التابعة للجهاز العصبي الودي , تعطى في حالات ارتفاع التوتر الشرياني و في حالات اضطرابات النظم القلبية.



توجد هذه المستقبلات في جدر الأوعية الدموية و في القلب و الكلى, و هي تتنبه بتأثير وسائط كيميائية ضمن الجسم \الأدرينالين\


إن عددها ليس ثابتا و إنما يتبع التوازن بين شدة التحريض بواسطة المنبهات \الأدرينالين\
و التنظيم الذاتي لها

فالاستعمال الطويل لحاصرات بيتا يزيد من عدد المستقبلات و بالتالي تزيد خطورة الإيقاف المفاجئ
إذ تجد المنبهات عددا كبيرا من المستقبلات لتؤثر فيها مما يجعل أثرها أشد قوة و خطرا.
و هذا ما يعرف بظاهرة الارتداد.


لدينا عموما نوعان من الظواهر الناتجة عن التوقف المفاجئ:




1- متلازمة نكس الأعراض:
تتصف لعودة أرقام الضغط الشرياني إلى ما كانت عليه قبل تطبيق العلاج أو بأرقام أعلى منها

و قد يكون ارتفاع الضغط صامتا \و هنا مكمن الخطر\ و لا يتجلى إلا بحادث وعائي شديد

كما قد يترافق بعلامات وظيفية مثل الصداع و اضطراب الرؤية


و من الناحية القلبية قد يتجلى بعودة نوبات الذبحة الصدرية الناجمة عن الجهد \\و التي تزول غالبا أثناء العلاج\\

كما قد تظهر أعراض غير ملاحظة سابقا \\نوبات ذبحة صدرية أثناء الراحة\\

و قد تحدث نوبات تسرع قلب شديدة أو احتشاءات.



2- متلازمة الحرمان:

تتصف بأبسط أشكالها بفقدان المريض لقدرته على القيام بأي مجهود فيعجز مثلا عن صعود السلالم
و لا يتمكن من القيام بالأعمال التي كان يمارسها سابقا

و في الحالات الشديدة يمكن أن نشاهد نوبات من القلق و الهياج و الاختلاط العقلي.



و مهما كان أمر هذه الأعراض تبقى أقل خطورة من متلازمة النكس التي ذكرناها بداية.



يمكن للأعراض التالية للتوقف أن تظهر مباشرة في اليوم التالي كما قد تتأخر حتى 20 يوما بعد ترك الدواء.

و تعالج بإعطاء المريض الدواء مرة أخرى إضافة إلى المهدئات إن اقتضى الأمر.






الباربيتورات:\أدوية الصرع\:

السيطرة على الصرع:

تهدف معالجة الصرع إلى منع حدوث النوبات من خلال المحافظة على تراكيز فعالة من الدواء المضاد للصرع في البلازما، لذا يعد ضبط الجرعة بعناية أمراً ضرورياً، ويجب أن تبدأ المعالجة بجرعات صغيرة تزاد تدريجياً إلى أن تتم السيطرة على النوبات أو تظهر تأثيرات فرط الجرعة.


يعتمد تحديد تواتر الجرعة غالباً على نصف عمر الدواء، ويجب أن تتم المحافظة عليه بأقل مستوى ممكن لتشجيع المريض على الامتثال للمعالجة. تعطى أغلب مضادات الصرع عند استخدامها بالجرعات الوسطية مرتين/يوم،

أما الفينوباربيتال Phenobarbital فيمكن إعطاؤه بجرعة مفردة عند النوم نظراً لنصف العمر الطويل له، ويمكن إعطاء الجرعات العالية مقسمة على 3 جرعات/يوم لتجنب حدوث الآثار الجانبية المرافقة للتراكيز البلازمية العالية.


تجدر الإشارة إلى أن استقلاب مضادات الصرع لدى الأطفال الصغار يكون أسرع منه لدى البالغين، ومن هنا يكون من الضروري إعطاء الجرعات بتواتر أعلى وبكميات أكبر نسبةً إلى وزن الجسم.



عموما تعد الباربيتورات مركبات تستخدم في حالات الأرق كمساعدة على النوم و بعضها يستخدم لعلاج الصرع كما تعطى كمهدئات في حالات القلق و الخوف.



يسبب الإيقاف المفاجئ متلازمتي نكس الأعراض و الحرمان بشكل متداخل, مثلا:

كل نوبة صرع تحدث بعد إيقاف الفينوباربيتال لدى أحد الأشخاص المصابين بالصرع تعد نكسا , أما لدى شخص غير مصاب به أساسا فهي متلازمة حرمان.


يؤدي الامتناع عن الباربيتورات في الحالات الخفيفة إلى القلق و الرجفان و الأرق , و تحصل النوبة الصرعية في الحالات الشديدة و تأخذ شكل نوبة مستمرة .


يفقد المريض أحيانا القدرة على التعرف على المكان و الزمان و تحصل ارتعاشات و فرط تعرق و ارتفاع حرارة , و تنتهي الحالات الشديدة بالسبات أ و الاضطرابات النفسية الحادة.



يكون المريض غالبا مسؤولا عن الإيقاف المفاجئ إذ ينسى أو يهمل المعالجة و قد يسبب إيقاف الدواء و لو ليوم واحد حدوث النوبة الصرعية خاصة في حالات الصرع الشديد التي تتطلب مقادير كبيرة من الدواء
ربما لا يكون الطبيب على علم بسوابق المريض و يجهل أنه يتعاطى مضادات الصرع فيعزي أسباب الهياج و القلق إلى أسباب أخرى بعيدة عن السبب الحقيقي.


تبدأ الأعراض بالظهور بصورة مبكرة من الأيام الأولى للحرمان إذا كان المركب قصير الأمد و تتأخر لعدة أيام في المركبات المديدة الأثر و لكنها تكون أشد.


العلاج يكون بإعادة إعطاء الباربيتورات و يضاف لها بعض المهدئات .
